Our Mission
Our mission is to help older adults in rural Mississippi stay healthy, safe and independent in their own homes for as long as possible. We do that by providing close-to-home primary care built on connection with each patient, their family caregivers and our community.

Our Services
Primary Care for Older Adults: Whole-person primary care focused on the needs of seniors, including annual wellness visits and advance care planning. Our nurse practitioner takes time to know each patient and their family.
Chronic Care Management & Remote Patient Monitoring: Monthly check-ins between visits for patients living with ongoing conditions like diabetes, heart disease and high blood pressure. Eligible patients can share blood pressure and blood sugar readings with us from home.
Home Visits for Homebound Seniors: For patients who can no longer easily leave home, our nurse practitioner comes to them. Family caregivers can be right there to ask questions and plan care together.
